Why are we collecting your personal information?

The Ministry of Health is collecting your personal information through this website. We are committed to protecting your privacy and managing your personal information in line with:

  • Health Records and Information Privacy Act 2002 (NSW)
  • Privacy and Personal Information Protection Act 1998 (NSW)

The purpose of the collection is to enable the delivery of the Challenge to Change program - a free, community-led health and wellbeing program for Aboriginal communities across NSW, supporting local teams to plan and deliver local activities that promote healthier lifestyles.

What personal information do we collect?

We will collect the following information from you:

  • name
  • email address
  • phone number
  • Aboriginality (Information about Aboriginality is collected to support culturally appropriate program design and delivery, and is handled with additional protections under NSW privacy legislation.)
  • residential postcode

How will we use and disclose this information?

The Ministry of Health will use the personal information we collect about you to:

  • Assess your team's suitability for the Challenge to Change program
  • Deliver and tailor the Challenge to Change program to your needs
  • Monitor your progress and provide support
  • Evaluate the effectiveness of the program
  • Conduct quality improvements
  • Communicate with you about the program, including reminders and updates

We may disclose this information with program delivery partners to support program delivery by posting program delivery resources to you.

Are you required by law to provide us with this information?

Providing us with the requested information is not required by law. However, if you choose not to provide us with the requested information, we may not be able to support your participation in the Challenge to Change program.

Is the information stored securely?

Yes. Information is stored in an appropriately secure way and held by the NSW Ministry in accordance with NSW Health's records management policy and securely deleted once no longer required.

Your record of visitation to the website

When you visit this site, our computers make a record of your visit and log the following information:

  • the IP (Internet Protocol) address of the machine which has accessed it
  • your top-level domain name (for example .com, .gov, .edu, .au, .uk etc.)
  • the address of your server
  • the date and time of your visit to the site
  • the pages accessed and documents downloaded
  • the previous site visited and
  • the type of browser and operating system you have used.

No personal identifiers are collected via cookies. We do not use marketing or third-party tracking cookies. Analytics tools are used only to monitor site performance and usage trends in accordance with NSW Government digital policies.
